Eligibility

At Peninsula Nursing and Rehabilitation, we warmly welcome candidates who meet the eligibility criteria for nursing home placement, sub-acute care, or rehabilitation services, provided they are at least 18 years of age. Our admissions team is dedicated to ensuring that each individual who seeks care with us receives the appropriate level of support and services tailored to their specific needs. To begin the admission process, our team will collaborate with you to assess your eligibility for care at our facility. This may involve reviewing your medical history, conducting a comprehensive assessment of your current health status, and evaluating your potential for healing and rehabilitation with us.

As part of the admission process, we require candidates to complete an application, as well as a Patient Review Instrument and Screen to help us better understand your care needs and goals. Our admissions team will guide you through each step of the process, providing support and assistance as needed to ensure that all necessary documentation is completed accurately and efficiently. If you are interested in Medicaid coverage, we have a dedicated Medicaid Coordinator who can help you determine your eligibility for Medicaid benefits and assist you with the application process.
